Attached for your approval is a resolution to approve Amendment No. 1 to the Professional Services Agreement (PSA) with Materials Testing Consultants, Inc. (MTC) for Material Testing Services for the 2018 Miscellaneous Utilities Projects.
In January 2018, the City received 6 proposals in response to RFP No. 18-02 for material testing services in the 2018 and 2019 construction season. A staff committee reviewed these proposals based on:
• Professional Qualifications (15%)
• Proposed Work Plan (25%)
• Past Performance (40%)
• Fee Proposal (20%)
The committee allocated testing work for the 2018 and 2019 construction seasons among 5 firms. MTC was selected to perform testing on the 2018 Miscellaneous Utilities Project.
Testing services requested by staff from MTC during the 2018 construction season used the initial authorized amount of $24,650.00 for the following work:
• Sanitary sewer on Stone School Road, Ticknor to Packard
• Sanitary sewer at Hikone, Packard Rd
• Water Main on Brookwood, White St to Packard
• Water Main on Sycamore, White St to Park Place
• Sanitary Sewer on Lenawee and Lafayette
Due to weather, some of the work was suspended until spring 2019:
• Sanitary sewer on S Forest, Roosevelt to Horman Ct.
• Sanitary Sewer and Water Main on Horman Ct, S Forest to Olivia
Budget and Fiscal Impact: The estimated fee to complete the project is $15,350.00, funding for which is available within the existing project budget. MTC complies with the requirements of the City’s Non-Discrimination and Living Wage ordinances.

Whereas, Materials Testing Consultants, Inc. (MTC) provided construction testing services for the 2018 Miscellaneous Utilities Projects per a professional services agreement (PSA) dated June 5, 2018;
Whereas, Construction testing for the utility work on S Forest Avenue, and on Horman Court, will be completed in 2019 under terms of the PSA; and
Whereas, Materials Testing Consultants, Inc. complies with the City’s Non-Discrimination and Living Wage Ordinances;
RESOLVED, Amendment No. 1 to the Professional Services Agreement with Materials Testing Consultants, Inc. for Material Testing Services for the 2018 Miscellaneous Utilities Projects in the amount of $15,350.00 be approved for a total authorized amount of $40,000.00;
RESOLVED, That the Mayor and City Clerk be authorized and directed to execute said amendment, after approval as to form by the City Attorney and approval as to substance by the City Administrator;
RESOLVED, That the City Administrator be authorized to take the necessary administrative actions to implement this resolution.
